Patient Financial Policy
The scope of our Financial Policy:
Courtagen’s financial policy encompasses the entire chain of events from insurance pre-authorization to final payment and account closure. Please note that Courtagen is primarily an out-of-network Provider. Please read and understand our cancellation policy. The specific areas of financial policy focus are:

Non-participation in Medicare / Medicaid / TriCare
Courtagen does not currently participate with Medicare, TriCare, and state Medicaid programs.
Blue Cross/Blue Shield Patients
Courtagen is considered an out-of-network provider with Blue Cross/Blue Shield. It is the policy of Blue Cross/Blue Shield to send payments directly to the patient. If you do receive a check from Blue Cross/Blue Shield that is reimbursement for the genetic testing Courtagen conducted, it is your responsibility to forward the check to Courtagen.
Courtagen will perform a pre-authorization step on behalf of a patient when required, typically when the patient has an HMO. It can take anywhere from a few days to a few weeks before the insurance plan will give us their response; every insurance plan is different. Once the insurance plan replies and gives us either a positive, negative or undetermined coverage response, then Courtagen proceeds forward based on the response given.
- If the insurance plan tells us they will cover the services, we proceed with processing your DNA sample. During this pre-authorization sometimes the insurance plan will notify us of any co-pays or deductibles that the patient will be responsible to pay.
- If the insurance plan tells us they will not cover any portion of the services, will only cover a very small amount, or will not determine at this time the coverage amount, then we will contact the patient to discuss available options.
The patient is responsible for all co-pays, deductibles, and unpaid balances, within the confines of federal and state law.
Self-Pay patient considerations
Some patients do not have insurance and, therefore, are considered Self-Pay patients. You may also become a Self-Pay patient if your insurance plan refuses coverage and you proceed with genetic testing. Courtagen will collect additional payment information from the patient to ensure that they can pay for the genetic testing services that will be provided. There are a range of payment options available, from upfront payments with discounts for paying early, credit card payment for up to 12 months and external financing options.
Courtagen allows patients to use multiple methods of payment, or to extend their payment into the future if need arises. We accept checks, credit cards, debit cards, wire or ACH payments. In certain circumstances, Courtagen allows patients to setup installment payment plans over a maximum of 12 months. Courtagen has a relationship with an external financing company that provides flexible medical-bill loans to eligible patients. We regret that a cancellation request received once a patient’s sample is in process cannot be honored and results will be reported to the ordering physician and billed to the appropriate party. We ask that any cancellation requested be emailed by the ordering physician to email@example.com.
Institutional billing accounts are accounts related to physician practices, hospitals and other laboratories for the purpose of research sample processing. Courtagen is CLIA certified and CAP accredited and we are happy to accept research samples and projects.
